
Connecticut Advance Directive Form
A Connecticut advance directive is a legal document that enables an individual to appoint a health care agent who can make medical decisions on their behalf if they are unable to communicate. Additionally, this document provides the individual the option to refuse life-sustaining treatments, such as ventilators, as well as food and hydration, should they become permanently unconscious.
