
Florida Advance Directive Form
An advance directive in Florida is a set of documents designed to specify a person's healthcare preferences and designate an agent to make decisions on their behalf if they are unable to communicate. This directive also includes instructions related to end-of-life care and organ donation wishes. After being signed and witnessed by at least two individuals, it becomes effective right away.

Chapter 765 (Health Care Advance Directives)"Advance directive" means a witnessed written document or oral statement in which instructions are given by a principal or in which the principal's desires are expressed concerning any aspect of the principal's health care or health information, and includes, but is not limited to, the designation of a health care surrogate, a living will, or an anatomical gift made pursuant to part V of this chapter.<a class="source-url" href="#source-url2">[2]</a>
