Utah power of attorney documents enable an individual ("agent") to manage financial or healthcare responsibilities on behalf of another person. This arrangement is often utilized by seniors who may find themselves unable to make decisions independently. Once the document is signed, it can be executed right away and does not need to be submitted to any governmental body.

The medical type of POA allows a person to appoint another to handle his or her health care decisions, if and when, it is clear that the principal can no longer communicate their wishes regarding treatment.
Signing Requirements: One disinterested adult witness.
Durable POA

A durable form is called "durable" because the agent appointed under it can continue to act, even after the person who created the POA is no longer able to make his or her own decisions.

General POA

A general form is used to convey broad financial powers to another to act on behalf of and in the best interest of the person who created the document (the principal) until it is revoked or the principal becomes incapacitated.

Limited POA

The limited form is used for more narrow circumstances such as a single transaction or a short period of time, where the principal needs someone to represent them temporarily.

Minor POA

A parent may choose someone else to be able to handle the everyday care of their child for a period of up to six (6) months.
Signing Requirements: No specific signing requirements; however, the document should be signed in the presence of a notary public.
Real Estate POA

Allows an individual to select a close friend, family member, attorney, or another person to handle real property decisions in their best interest.

Revocation of POA

The revocation form is used when there is a pre-existing POA that the principal wants to terminate.

This form is used in the event that a person needs to allow another person, usually a tax professional, access to their filings so that they can help them with issues involving the tax authority.
