The Ohio Department of Health, Vital Statistics Office manages birth and death records, as well as abstracts of marriage and divorce records.Death records are available from January 1, 1954 to present. Record requests can be made in person, by mail, or online. Each county also has a local health department where records are managed for births and deaths that occurred in that county.

Search the Ohio History Connection death certificate records index by name, date span, and county. Available records include Ohio Department of Health death certificates from 1914-1944 and 1954-1963; Ohio Department of Health Stillborn death certificates from 1913-1935 and 1942-1953; and Columbus Board of Health death certificates 1904-1908. Copies can be printed in the Archives & Library or ordered online for a fee.
